I state that, to the best of my knowledge, I have no current medical condition that would prohibit me from participating in strength, aerobic, and/or flexibility activities. I further hold York Club Fitness, its agents and employees, harmless for any injury, damage or loss to my person and/or property sustained while in the club, except those due to negligence.*

I understand that membership cancellation or freeze requires a 30-day notice and that a $60 semi-annual maintenance fee will apply starting from my one-year anniversary.*

By submitting this Agreement, I confirm that I am enrolling as a member of York Club Fitness and that my membership is considered confirmed upon submission. I authorize York Club Fitness and its authorized representatives to contact me by phone to obtain, verify, or update any required billing information necessary to activate and maintain my membership account. I understand that failure to provide valid billing information may delay activation of my membership or affect my access to the club.*
